Consent and Confidentiality
Please be advised that following extensive consultation including taking expert legal advice, the NTA has announced some changes in the recommended wording for collecting and discussing informed consent for sharing treatment data with NDTMS.
The attached documents include a letter from the NTA detailing the reason for this change, the guidance with the new wording (changes have occurred on help sheets 3, 4, 7 & 8) and see below for the form required to be returned by your service in hardcopy form.
Note: the consent document may be amended by regional NDTMS teams and/or DAT wide data sharing agreements to include additional consents (e.g. local clinical record sharing), but should at a minimum include the information contained in this guidance.

From 1st September 2009 it will be assumed that all new clients presenting for treatment at your agency will be made aware of this revised information either through your own documentation incorporating this new guidance and/or through verbal discussions. The document itself is self-explanatory as to why these changes have been made.
